Incident Reporting - Ports

Please utilize this form to report any incident related to activities in the South African Oil and Gas / Ship Repair industry in the port environment. Consider as an incident any uncontrolled or unplanned event, or sequence of events, that results in a fatality, injury, financial loss or near miss.

Submitted forms will not be published anywhere, they will be used to track severity and type of incident in the ports to inform SAOGA where there are deficits, where training, further regulations and awareness is required.
